Sir,

The authors have included most patients with one-stage palate closure when research has proven that primary cleft palate repair produced growth inhibition.[1] The authors have not explained the nature of unilateral cleft (whether complete or incomplete). This is important as early soft palate closure reduces the width of the cleft and performing two-stage palate repair (soft palate repair and delaying hard palate closure) achieved good facial growth [2],[3],[4] and had a smaller effect on the maxillary growth compared to one-stage palate repair.[5],[6],[7]

The sample group of cleft palate chosen had a wide mean age (10–18 years irrespective of gender), especially when unilateral (complete) cleft lip and palate repair using von Langenbeck technique at 4–12 years produced better head-face morphology.[8]

The addition of objective to compare (early vs. late palatoplasty) effect on mandible morphology and spatial position was novel effort by authors. However, the cephalometric parameters chosen by authors were mandibular body length, ramal length, and gonial angle when earlier research had proven a shorter ramus and body length, with a backward mandibular rotation and an obtuse gonial angle.

Future research could be focused on the spatial position of the mandible using cone beam computed tomography to assess the glenoid fossa position, ramal inclination, vertical position of the condyle, changes in the inclination of the temporomandibular fossa, and chin position after unilateral cleft repair.
